How do I create a ticket request

To create a Ticket request:

- Select 'Forms' menu at top of screen
- Select 'Ticket request' at end of 'Forms' menu
- Click 'Request New Assessment' button

- Select post from 'Post' drop down list
- Select check box for required form
- Click 'Next' button
- Enter 'Assessor Email'
- Click 'Next' button and add comment for assessor if necessary
- Click 'Finish' button

 

 

To request a signed procedure

 
- Select 'Forms' menu at top of screen
- Select 'Ticket request' at end of 'Forms' menu
- Click 'Request New Assessment' button
- Select Training period  from 'Post' drop down list
- Select check box for required procedure
- Click 'Next' button
- Enter 'Assessor Email'
- Click 'Next' button and add comment for assessor if necessary
- Click 'Finish' button

 

Please introduce yourself to the person you are asking to assess you and remind them of the procedure / event you are asking them to create the assessment about.
